چکیده

Background Although it prevents severe forms of the disease, vaccination does not completely protect against occurrence COVID19 disease. If, DMARDs used have been associated with variable humoral response to SARS-CoV-2 vaccination, impact their use after natural infection poorly studied. Objectives To characterize and viral persistence in nasopharyngeal sphere (NP), stools blood patients rheumatic disease under DMARDs, compared healthy controls. Methods Prospective monocentric longitudinal study including rheumatoid arthritis or spondyloarthritis a confirmed (positive NP PCR and/or positive serology pathognomonic thoracic tomography (CT)) during first second wave COVID pandemic. Patients were followed up until one year was assessed before vaccination. Serum IgG IgA antibodies spike (S) nucleocapsid (N) proteins measured at every visit. Viral early visit using conventional RT-PCR high sensitive technique (droplet digital PCR). Results Between June 2020 July 2021, we include 96 (50 SpA 46 RA) mean age 53 +/- 14 years 20 controls (mean 49 ± 16 years) corresponding relatives (spouses, children) living together infected same time. The immune responses analyzed according 6 treatment groups: methotrexate (MTX)/salazopyrine (SLZ) monotherapy (n=17/2); anti-TNF (n=24), + MTX (n=23); rituximab (RTX) (n=11); anti-IL17 -23 (n=8); others (n=11). Visits made 1 month (29 ±13 days; n=18), 3 months (110 ±23 n=67), (231 ±35 n=48) 12 (368 19 n=19) infection. anti-S anti-N Ab titers significantly different groups control population months. A significant decrease noted group treated RTX (p=0.007) molecules targeting IL17/23 pathway (p=0.007). similar but non-significant trend observed these for (p=0.07). anti-SARS-CoV-2 M3, Detection SARS-Cov-2 RNA serum negative all samples taken 4 (2 RA abatacept/RTX 2 anti-TNF/secukinumab) had low very load 1-month Ct 36). None form Conclusion - previous proven COVID-19 did seem alter anti-IL17/-IL-23 might response. This lower more In patients, new may be considered as full boost system. induce serum, stool. Interpretation of Serologic Test Results and Evaluation of Humoral Immune Response After Vaccination Following Infection with COVID-19 Disease

SARS-CoV-2 virus infection induces a cellular and humoral mediated immune response and produces antibodies against viral antigens such as nucleocapsid (N) protein and spike (S) protein. Anti-protein S antibodies also target the spike protein subunit S1 and the receptor binding domain (RBD). Serologic tests can detect the presence of these antibodies in blood serum within a few days to a few wee...

CHALLENGES OF LABORATORY SAMPLING AND DIAGNOSIS OF SARS-COV-2 VIRUS OF DISEASE (COVID-19)

Background & Aims: Given the prevalence of SARS-CoV-2 worldwide, it is essential to identify people infected with the virus and determine its different types to control the global outbreak of COVID-19. The results of the studies are controversial, so this study examines these diagnostic challenges, including the types of diagnostic methods, the type and time of sampling, and even the clinical c...

SARS-CoV-2 infection: its psychiatric manifestations

Background:  The clinical presentation of SARS-CoV-2 infection was initially dominated by respiratory symptoms. However the clinical spectrum is wide and neuropsychiatric syndromes are also a source of medical concern. Our aims are to present an atypical clinical presentation of SARS-CoV-2 infection characterized by auditory hallucinations and unusual behavior and to emphasize the diversity of ...

Angiotensin-Converting Enzymes, the Key Components in the Pathogenesis of COVID-19 Infection and their Role in the Interaction of SARS-Cov-2 with Human Host Cells

Introduction: Over the past 20 years, seven coronaviruses have caused more or less severe respiratory diseases in humans. Among these, the most important ones are SARS-CoV and the coronavirus, a similar virus that has created a pandemic called Covid-19 since 2019, belonging to the b-category of beta-coronaviruses called Sarbecovirus. This virus is due to a kind of spike-like structure to the A...
